Resumo

Introduction: Spirituality is increasingly recognized as an important dimension of comprehensive care, particularly in chronic illness and end-of-life settings. Primary healthcare provides a favorable context for addressing these needs through longitudinal and patient-centered care, although its practical integration remains insufficiently described. Case description: A 77-year-old female patient with multiple comorbidities, including advanced heart failure (NYHA IV) secondary to Chagas disease, coronary artery disease with prior myocardial infarctions and stent placement, chronic kidney disease, and depression, presented with progressive functional decline and recurrent hospitalizations. The patient lived in a socially vulnerable context, with financial limitations and family conflicts. Spirituality was a central component of her life, characterized by daily prayer, active participation in a religious community, and regular visits from Eucharistic ministers. During disease progression, spirituality functioned as a coping mechanism, supporting meaning-making, emotional regulation, and acceptance of illness and finitude. Initial care was fragmented, but primary healthcare later assumed a central role, providing longitudinal follow-up, home visits, and a palliative care approach. Pastoral support was integrated into the care process, acting in a complementary manner to clinical management. The family physician maintained continuity of care, including during hospital admissions. The patient experienced progressive clinical deterioration and died in a hospital setting, and bereavement support was provided to the family. Data were collected from the patient’s medical record and narrative accounts within a project approved by an institutional ethics committee. Conclusion: This case highlights spirituality as a central dimension in the experience of illness and demonstrates how collaboration between primary healthcare and pastoral support can contribute to comprehensive, person-centered care that respects human dignity. Integrating spiritual dimensions into clinical practice may strengthen ethical, relational, and holistic approaches to care, particularly in advanced illness, reaffirming the role of medicine not only in treating disease but also in accompanying the person.
