Introduction: Cancer represents a significant global health challenge, with a high proportion of patients experiencing clinically significant psychosocial and spiritual distress. Contemporary oncology emphasizes a whole-person approach, acknowledging that addressing the spiritual dimension is a core component of holistic, palliative, and supportive care. For many, a diagnosis initiates a profound journey involving questions about meaning, purpose, and hope, making spirituality a critical determinant of health. Objective: This study aims to analyze the role of spirituality in the care delivery and clinical outcomes of oncology patients. Methods: A systematic literature review was performed using the PubMed database with the descriptors “spiritually” AND “cancer”. Filters were applied for systematic reviews and reviews published between 2021 and 2025. The selection and screening process was conducted with the aid of Rayyan software to ensure methodological rigor. Results: The literature identifies spirituality as a central coping resource significantly linked to improved health-related quality of life and greater resilience throughout the illness trajectory. Research consistently shows that spiritual well-being is associated with reduced psychological suffering, including lower levels of anxiety, depression, stress, and hopelessness. Furthermore, evidence indicates a positive impact on treatment adherence and general well-being. In the context of advanced disease, spiritual support is associated with better end-of-life outcomes, such as higher rates of hospice utilization and improved quality of life near death. Conclusion: Spirituality constitutes an indispensable component of integral oncology care, with clear applicability in clinical practice through spiritual screening, assessment, and interdisciplinary support. Integrating this dimension into standard protocols is essential for delivering patient-centered care and optimizing psychosocial clinical outcomes.
