Summary
Febrile neutropenia, characterized by episodes of fever and low neutrophil count, is one of the most frequent life-threatening complications in oncologic patients undergoing treatment. In order to evaluate possible viral causes for febrile neutropenia, we applied viral metagenomics, on blood and oropharyngeal swab samples obtained from 15 pediatric patients with oncological diseases and febrile neutropenia from the Onco-Pediatric outpatient clinic of the Hospital das Clínicas, Faculty of Medicine of Ribeirão Preto, University of São Paulo. A control group consisting of 15 pediatric patients with oncological diseases in treatment was also collected. Samples were sequenced in a Illumina NextSeq 1000/2000 platform. Data processing was performed utilizing the following pipeline: quality control with FastQC v.0.11.8, trimming and adapter removal with Fastp v.0.23.1, removal of human genome reads with Burrows-Wheeler Aligner software and Kraken2 v.2.0.8 to infer taxonomic classification. Viral abundance was estimated based on the total number of raw read counts obtained and results were transformed into tables plotted in bar graphs using the RStudio environment v.4.2.3, ggplot2 package. We observed high abundance of commensal viruses and especially anelloviruses in both groups, excluding nasopharyngeal swabs obtained from neutropenic patients. We found several viruses of clinical relevance in patients with febrile neutropenia including Adenovirus, Herpes Simplex Virus (HSV), Beta Herpesvirus 5, 6 and 7 (HHV-5, 6 and 7), Epstein Barr Virus and the polyomaviruses Malawi (MW) and Washington University (WU), all of which were confirmed by direct molecular techniques. Establishing a direct relationship between the identified viral agents and neutropenic episodes poses challenges; however, this study illuminates potential viral causes, thereby enhancing our comprehension of the etiology of this severe condition in oncologic patients. Such insights may prove pivotal for the appropriate management and treatment of patients.
