Resumo
Objective: To report the care experience of a child with toxic epidermal necrolysis (TEN), using a specialized treatment plan with antimicrobial and anti-inflammatory dressings prescribed by the nurse. Methods: This is a descriptive case study of a 5-year-old child with autism spectrum disorder and epilepsy, admitted to the ICU after developing a drug reaction due to a change in antiepileptic medication (valproic acid). After a thorough evaluation of the skin lesions, a care protocol was developed based on current wound care guidelines. The selected dressings included: 0.1% polyhexanide biguanide (PHMB) solution for cleansing, tea tree and copaiba oil gel, rayon gauze with tea tree and copaiba oil, and tubular mesh. Dressing changes were performed every 72 hours, with photographic and descriptive records maintained over a 10-day treatment period. Results: TEN is a rare and severe condition, characterized by adverse reactions to medications, potentially leading to fatal complications. The nurse plays a fundamental role in assessing and selecting appropriate wound dressings. In this case, progressive improvement of the lesions was observed at each dressing change. The materials used proved effective in several aspects: infection prevention, pain relief, atraumatic dressing changes, and cost reduction. The PHMB solution and tea tree/copaiba gel demonstrated antimicrobial and anti-inflammatory efficacy, while rayon gauze and tubular mesh facilitated handling and ensured patient comfort. Conclusion: The nurse’s role was essential to the patient’s recovery, particularly in selecting appropriate dressings and implementing an effective treatment plan. The specialized approach and continuous documentation enabled ongoing evaluation of clinical progress and complication prevention. This case highlights that combining antimicrobial and anti-inflammatory dressings with skilled nursing care can significantly contribute to therapeutic success in pediatric TEN cases.
