Resumo
Objective: To report the case of a patient with a superficial second-degree burn treated with lyophilized tilapia skin as a biological dressing, highlighting its efficacy in wound healing. Methods: This is a descriptive case report study conducted in a burn treatment center at a tertiary-level public hospital located in Fortaleza, Ceará, Brazil. The follow-up consisted of four visits. The first visit (V1) involved the initial evaluation and application of the biological dressing. Subsequent visits (V2 and V3) occurred at five-day intervals, and the last visit (V4) was scheduled seven days after wound healing to provide post-treatment guidance regarding the use of tilapia skin. The study was approved by the Research Ethics Committee of the Federal University of Ceará under opinion number 3.285.193. Results: The patient, a 26-year-old male, identifies as mixed-race, single, with a completed elementary education, no comorbidities, no history of previous illnesses, allergies, or medication use, was admitted to the outpatient burn clinic after sustaining a superficial second-degree thermal burn due to scalding on the left lower limb (thigh) and abdomen, covering 6% of the total body surface area. During the first visit (V1), after assessing the injury, the patient was referred for treatment with lyophilized tilapia skin. The skin was hydrated with 0.9% saline solution for 15 minutes before application, and the burned area was cleansed with running water and 2% chlorhexidine digluconate. Six overlapping fragments of tilapia skin were applied, extending 1 cm beyond the wound margins, followed by a secondary dressing using gauze and bandages. Written instructions on home care and analgesic use were provided. After five days (V2), the patient returned for a follow-up evaluation. The secondary dressing was removed, revealing adhesion of the biomaterial to the burned areas. A new secondary dressing was applied, and a follow-up appointment was scheduled in five days. At visit 3 (V3), ten days after the initial application, the lyophilized tilapia skin had become dry and hardened, leading to its natural separation from the healed area. The remaining skin was removed using running water. After seven days (V4), the epithelialized region was evaluated, a moisturizing cream was applied, and further instructions were given to the patient. Conclusion: Lyophilized tilapia skin proved to be an effective alternative for treating superficial second-degree burns, promoting wound healing within ten days, reducing pain, and minimizing dressing changes. This approach improves the patient’s quality of life during treatment while also reducing healthcare service costs.
