Summary
Background: The risk of transfusion transmission of Hepatitis A Virus (HAV) has been described in several countries. Recent reports demonstrated an increase in the rate of HAV-RNA detection among blood donors after epidemic outbreaks periods of hepatitis A (HA), in France. In Brazil, these data are still scarce. Objective: The aim of this study was to evaluate whether the occurrence of outbreaks of HA in Rio de Janeiro influences the incidence of HAV among blood donors and to characterize the circulating strains of HAV. Methods: For this study, 41 plasma samples collected in 2018 from blood donors from the city of Rio de Janeiro and 42 plasma samples from blood donors living in Vidigal and Rocinha neighborhoods, Rio de Janeiro, where outbreaks of HA were reported from 2017 to 2018, were analyzed. The detection of HAV-RNA was performed by Nested-PCR and anti-HAV IgM was evaluated by ELISA. Results: HAV-RNA was detected in 4/42 (9,5%) samples from blood donors living in Vidigal and Rocinha, and in 2/41 (4,8%) samples from blood donors who live in other locations in the city of Rio de Janeiro. These results indicate that the occurrence of the hepatitis A outbreak increased the prevalence of HAV among blood donors. All blood donors were anti-HAV IgM negative, suggesting a period of immunological window in these individuals at the time of donation. Most donors infected were male (55%) aged between 20 and 30 years (47%).These data are similar to the latest epidemiological bulletin (2018) which reports an increase in the rate of hepatitis A incidence in men aged 20 to 39 years in recent years. Among the four HAV-RNA positive samples from donors living in Vidigal, two were classified as subgenotype IA and two as IB, which have an endemic circulation pattern in Rio de Janeiro. Conclusion: According to previous studies, in Brazil and other countries the increase in the incidence of hepatitis A has been reported due to the occurrence of large outbreaks. These results highlight the importance of knowing the incidence and prevalence of this infection among blood donors and of assessing the impact of epidemic outbreaks on the risk of transfusion transmission.
