Resumo
INTRODUÇÃO: A imunoterapia revolucionou o tratamento oncológico nas últimas décadas, representando um dos maiores avanços da medicina contemporânea. Inibidores de checkpoint imune (anti-PD-1/PD-L1, anti-CTLA-4) e terapias celulares como CAR-T demonstraram eficácia em diversas neoplasias, especialmente hematológicas e alguns tumores sólidos. Entretanto, desafios ainda limitam sua aplicação em cerca de 90% dos tumores sólidos. Os principais obstáculos envolvem resistência tumoral, heterogeneidade antigênica – variação da expressão de antígenos entre células do mesmo tumor, favorecendo escape clonal – e microambiente imunossupressor, composto por células (Tregs, MDSCs e TAMs) e citocinas inibitórias (TGF-ß e IL-10) que comprometem a resposta imune local. Avanços em terapias celulares de nova geração, anticorpos multiespecíficos, vacinas personalizadas de mRNA e moduladores do microambiente tumoral ampliam as possibilidades terapêuticas. OBJETIVOS: Analisar os avanços recentes da imunoterapia no câncer, destacando mecanismos terapêuticos, inovações tecnológicas, desafios relacionados à resistência tumoral e perspectivas da medicina de precisão. METODOLOGIA: Revisão sistemática realizada nas bases PubMed, SciELO e Google Acadêmico. Utilizaram-se os descritores “Câncer”, “Imunoterapia” e “Tecnologias”, combinados pelos operadores AND e OR. Foram incluídos artigos publicados entre 2021 e 2026, disponíveis gratuitamente em texto completo e relacionados à imunoterapia no câncer. Estudos com acesso pago foram excluídos. Os artigos selecionados foram analisados e comparados para síntese das informações relevantes. RESULTADOS E DISCUSSÃO: A análise da literatura demonstra que a imunoterapia consolidou-se como um dos pilares da oncologia moderna. Diferentemente das abordagens tradicionais, ela potencializa respostas imunes antitumorais específicas e promove memória imunológica duradoura (Zhang et al., 2025). Entre as modalidades de maior impacto clínico destacam-se os Inibidores de Checkpoint Imune (ICIs), especialmente os direcionados às vias PD-1/PD-L1 e CTLA-4 (Wang et al., 2021; Rui et al., 2023). Esses agentes restauram a atividade citotóxica dos linfócitos T exauridos, contribuindo para respostas expressivas em neoplasias como melanoma avançado (Garg et al., 2024). Estudos clínicos demonstram que a eficácia dos ICIs pode ser ampliada quando associada a terapias convencionais. No câncer de mama triplo-negativo, a combinação de bloqueadores de PD-1/PD-L1 e quimioterapia melhorou significativamente os desfechos clínicos (Debien et al., 2023). Da mesma forma, a radioterapia em baixas doses remodelou o microambiente tumoral imunossupressor, aumentando a infiltração de células T e favorecendo a morte celular imunogênica (Zhang et al., 2025). Apesar dos avanços, a eficácia da imunoterapia em tumores sólidos permanece limitada por resistência tumoral, heterogeneidade antigênica e microambiente imunossupressor. A heterogeneidade favorece o escape imunológico e a seleção de clones resistentes, reduzindo a eficácia terapêutica (Shui et al., 2025). Além disso, Tregs, MDSCs e macrófagos associados ao tumor (TAMs) comprometem a atividade efetora dos linfócitos T (Garg et al., 2024). As células CAR-T apresentam resultados expressivos em neoplasias hematológicas refratárias, embora sua aplicação em tumores sólidos ainda enfrente barreiras relacionadas à infiltração tumoral, heterogeneidade antigênica e imunossupressão local (Garg et al., 2024; Rui et al., 2023). Paralelamente, anticorpos biespecíficos demonstram potencial para ampliar o reconhecimento tumoral e minimizar mecanismos de escape imunológico (Shui et al., 2025). As vacinas personalizadas de mRNA também emergem como estratégia promissora ao utilizar neoantígenos específicos para induzir respostas imunológicas individualizadas (Shariati et al., 2024; Laila et al., 2024). Entretanto, desafios relacionados à estabilidade do mRNA e aos sistemas de entrega intracelular ainda limitam sua aplicação clínica, impulsionando o desenvolvimento de nanopartículas lipídicas (LNPs). Persistem limitações importantes, incluindo eventos adversos relacionados à imunidade (irAEs), toxicidades inflamatórias e Doença Hiperprogressiva (HPD) (Wang et al., 2021; Shui et al., 2025). Nesse cenário, biomarcadores genômicos e transcriptômica de célula única (single-cell RNA-seq) tornam-se essenciais para prever respostas terapêuticas, identificar mecanismos de resistência e reduzir riscos de toxicidade (Garg et al., 2024; Zhang et al., 2025). CONCLUSÃO: A imunoterapia consolidou-se como estratégia fundamental no tratamento do câncer, promovendo avanços por meio dos inibidores de checkpoint imune, terapias celulares, anticorpos biespecíficos e vacinas de mRNA. Entretanto, desafios relacionados à resistência tumoral, heterogeneidade antigênica e microambiente imunossupressor ainda limitam sua eficácia, especialmente em tumores sólidos. A incorporação de biomarcadores genômicos e tecnologias associadas à medicina de precisão mostra-se essencial para otimizar respostas terapêuticas e ampliar as perspectivas de tratamentos personalizados. INTRODUCTION: Immunotherapy has revolutionized cancer treatment, representing one of the greatest advances in modern medicine. Immune checkpoint inhibitors (anti-PD-1/PD-L1 and anti-CTLA-4) and cellular therapies such as CAR-T cells have shown efficacy in several malignancies, especially hematological cancers and some solid tumors. However, challenges still limit their application in nearly 90% of solid tumors. The main barriers include tumor resistance, antigenic heterogeneity - variation in antigen expression among tumor cells that promotes clonal escape - and an immunosuppressive tumor microenvironment composed of regulatory T cells (Tregs), myeloid-derived suppressor cells (MDSCs), tumor-associated macrophages (TAMs), and inhibitory cytokines such as TGF-ß and IL-10. Advances in next-generation cellular therapies, multispecific antibodies, personalized mRNA vaccines, and tumor microenvironment modulators have expanded therapeutic possibilities. OBJECTIVE: To analyze recent advances in cancer immunotherapy, highlighting therapeutic mechanisms, technological innovations, challenges related to tumor resistance, and future perspectives of precision oncology. METHODOLOGY: Literature review conducted through PubMed, SciELO, and Google Scholar. The keywords “Cancer,” “Immunotherapy,” and “Technologies” were combined using the Boolean operators AND and OR. Articles published between 2021 and 2026, available in full text and free of charge, were included. Studies requiring paid access were excluded. Selected articles were analyzed and compared to synthesize the most relevant information. RESULTS AND DISCUSSION: Recent literature demonstrates that immunotherapy has become a pillar of modern oncology. Unlike traditional approaches, it enhances specific antitumor immune responses and promotes long-lasting immunological memory (Zhang et al., 2025). Among the modalities with the greatest clinical impact are Immune Checkpoint Inhibitors (ICIs), particularly those targeting PD-1/PD-L1 and CTLA-4 pathways (Wang et al., 2021; Rui et al., 2023). These agents restore the cytotoxic activity of exhausted T lymphocytes, contributing to significant responses in malignancies such as advanced melanoma (Garg et al., 2024). Clinical studies show that ICI efficacy can be enhanced when combined with conventional therapies. In triple-negative breast cancer, the combination of PD-1/PD-L1 inhibitors and chemotherapy improved clinical outcomes (Debien et al., 2023). Likewise, low-dose radiotherapy remodeled the immunosuppressive tumor microenvironment, increasing T-cell infiltration and promoting immunogenic cell death (Zhang et al., 2025). Despite these advances, immunotherapy effectiveness in solid tumors remains limited by tumor resistance, antigenic heterogeneity, and the immunosuppressive microenvironment. Antigenic heterogeneity promotes immune evasion and the selection of resistant clones, reducing therapeutic efficacy (Shui et al., 2025). Furthermore, Tregs, MDSCs, and tumor-associated macrophages impair T-cell effector activity (Garg et al., 2024). CAR-T cells have shown remarkable results in refractory hematological malignancies, although their application in solid tumors still faces barriers related to tumor infiltration, antigenic heterogeneity, and local immunosuppression (Garg et al., 2024; Rui et al., 2023). Likewise, bispecific antibodies have demonstrated potential to improve tumor recognition and reduce immune escape mechanisms (Shui et al., 2025). Personalized mRNA vaccines have also emerged as a promising strategy by using tumor-specific neoantigens to induce individualized immune responses (Shariati et al., 2024; Laila et al., 2024). However, challenges related to mRNA stability and intracellular delivery systems still limit clinical application, driving the development of lipid nanoparticles (LNPs). Important limitations remain, including immune-related adverse events (irAEs), inflammatory toxicities, and Hyperprogressive Disease (HPD) (Wang et al., 2021; Shui et al., 2025). In this scenario, genomic biomarkers and single-cell transcriptomics (single-cell RNA-seq) are essential to predict therapeutic responses, identify resistance mechanisms, and reduce toxicity risks (Garg et al., 2024; Zhang et al., 2025). CONCLUSION: Immunotherapy has become a fundamental strategy in cancer treatment, promoting advances through immune checkpoint inhibitors, cellular therapies, bispecific antibodies, and mRNA vaccines. However, challenges related to tumor resistance, antigenic heterogeneity, and the immunosuppressive tumor microenvironment still limit its effectiveness, particularly in solid tumors. The incorporation of genomic biomarkers and precision medicine technologies is essential to optimize therapeutic responses and expand the prospects for personalized treatments.
