Resumo
Introdução: A profilaxia pré-exposição ao HIV (PrEP) representa uma estratégia fundamental para a prevenção da infecção pelo HIV, demonstrando eficácia de 54-76% na redução da incidência quando há boa adesão ao esquema terapêutico. No Brasil, a PrEP foi oficialmente implementada pelo Sistema Único de Saúde (SUS) em 2018, marcando um avanço significativo nas políticas públicas de prevenção combinada ao HIV. Desde então, observou-se ampliação progressiva da oferta do serviço em todo o território nacional. Apesar desses avanços, persistem disparidades regionais significativas no acesso à PrEP, influenciadas por múltiplos fatores estruturais, socioeconômicos e organizacionais. Entre os determinantes identificados, destacam-se a cobertura e a capacidade instalada da Atenção Primária à Saúde (APS), a disponibilidade de serviços especializados, a infraestrutura tecnológica dos serviços de saúde, a qualificação profissional para prescrição e acompanhamento da PrEP, além de barreiras socioculturais como estigma, discriminação e baixa percepção de risco entre populações-chave. No contexto brasileiro, a região Sudeste tende a registrar maior captação e retenção de usuários de PrEP em comparação à região Nordeste. Paradoxalmente, o Nordeste apresenta maior cobertura populacional da Estratégia Saúde da Família (ESF), o que sugere que a simples presença de equipes de APS não é suficiente para garantir acesso equitativo a tecnologias de prevenção como a PrEP. Essa discrepância aponta para a necessidade de compreender melhor os mecanismos que facilitam ou dificultam a integração da PrEP na rotina da APS e a efetivação do acesso em diferentes contextos regionais. Objetivo: Comparar a distribuição de usuários de PrEP entre as regiões Sudeste e Nordeste do Brasil nos anos de 2023 e 2024, analisando a relação entre cobertura da Estratégia Saúde da Família e acesso ao serviço. Metodologia: Estudo descritivo com dados secundários do Painel PrEP (Ministério da Saúde) referentes a 2023 e 2024. Analisou-se o número total de novos usuários de PrEP nas regiões Sudeste e Nordeste, correlacionando com dados de cobertura da ESF (SISAB) e indicadores socioeconômicos (IBGE). Resultados: Em 2023, a região Sudeste registrou 63.629 usuários em PrEP, dos quais 32.499 eram novos usuários iniciados naquele ano. No mesmo período, a região Nordeste contabilizou 14.036 usuários em PrEP, sendo 7.940 novos usuários. Isso representa uma diferença absoluta de aproximadamente 49.593 usuários entre as duas regiões, com o Sudeste concentrando cerca de 82% do total de usuários quando comparado ao Nordeste. Em 2024, observou-se crescimento expressivo em ambas as regiões. O Sudeste alcançou 99.333 usuários em PrEP, com 48.615 novos iniciados, enquanto o Nordeste atingiu 22.323 usuários, com 11.464 novos usuários. Apesar do aumento absoluto em ambas as regiões, a proporção relativa manteve-se desfavorável ao Nordeste, com o Sudeste apresentando aproximadamente 77% mais usuários em números absolutos e uma taxa de crescimento proporcional superior. Quando analisada a cobertura da Estratégia Saúde da Família, observou-se que o Nordeste mantém cobertura populacional superior a 80%, enquanto o Sudeste apresenta cobertura em torno de 52%. Esse dado evidencia que, apesar da maior presença de equipes de APS no Nordeste, a captação e retenção de usuários de PrEP permanecem significativamente inferiores. Tal achado sugere que a infraestrutura tecnológica, a disponibilidade de serviços especializados, a capacitação profissional e determinantes socioculturais exercem influência mais significativa sobre o acesso à PrEP do que a cobertura formal de equipes da APS. Outros fatores que podem explicar essa disparidade incluem a concentração de centros de testagem e aconselhamento (CTA) e serviços de assistência especializada (SAE) em HIV/AIDS na região Sudeste, maior acesso a tecnologias de informação e comunicação para divulgação da PrEP, maior engajamento de organizações da sociedade civil na mobilização de populações-chave, e menor estigma associado à busca por serviços de prevenção ao HIV em contextos urbanos mais desenvolvidos. Esses resultados corroboram achados da literatura internacional que apontam para a existência de barreiras multinível ao acesso à PrEP, incluindo fatores individuais (percepção de risco, estigma, conhecimento sobre PrEP), comunitários (normas sociais, discriminação) e estruturais (disponibilidade de serviços, capacitação profissional, infraestrutura). É importante destacar que o crescimento observado em ambas as regiões entre 2023 e 2024 indica que as estratégias de expansão da PrEP estão produzindo resultados positivos. No entanto, o crescimento proporcionalmente maior no Sudeste sugere que as desigualdades podem estar se ampliando, reforçando a necessidade de políticas específicas para acelerar a implementação no Nordeste e em outras regiões com menor acesso. O monitoramento contínuo desses indicadores é fundamental para avaliar o impacto das intervenções e orientar ajustes nas estratégias de implementação. Conclusão: Apesar da maior cobertura da Estratégia Saúde da Família, a região Nordeste mantém acesso significativamente inferior à PrEP quando comparada à região Sudeste. Esse achado evidencia que a expansão quantitativa da APS não é suficiente para garantir acesso equitativo a tecnologias de prevenção ao HIV, sendo necessário investimento em qualificação profissional, infraestrutura, articulação intersetorial e enfrentamento de barreiras socioculturais. Introduction: Pre-exposure prophylaxis (PrEP) for HIV is a key strategy for preventing HIV infection, demonstrating an efficacy of 54–76% in reducing incidence when adherence to the therapeutic regimen is adequate. In Brazil, PrEP was officially implemented by the Unified Health System (SUS) in 2018, representing a significant advancement in public policies for combination HIV prevention. Since then, a progressive expansion in service availability has been observed throughout the country. Despite these advances, substantial regional disparities in access to PrEP persist, influenced by multiple structural, socioeconomic, and organizational factors. Among the identified determinants are the coverage and installed capacity of Primary Health Care (PHC), the availability of specialized services, the technological infrastructure of health services, professional training for PrEP prescription and follow-up, and sociocultural barriers such as stigma, discrimination, and low risk perception among key populations. In the Brazilian context, the Southeast region tends to show higher PrEP uptake and retention rates compared with the Northeast region. Paradoxically, the Northeast has greater population coverage by the Family Health Strategy (FHS), suggesting that the mere presence of PHC teams is insufficient to ensure equitable access to prevention technologies such as PrEP. This discrepancy highlights the need to better understand the mechanisms that facilitate or hinder the integration of PrEP into PHC routines and the effective provision of access across different regional contexts. Objective: To compare the distribution of PrEP users between the Southeast and Northeast regions of Brazil in 2023 and 2024, analyzing the relationship between Family Health Strategy coverage and access to the service. Methodology: A descriptive study was conducted using secondary data from the PrEP Dashboard (Brazilian Ministry of Health) for the years 2023 and 2024. The total number of new PrEP users in the Southeast and Northeast regions was analyzed and correlated with Family Health Strategy coverage data (SISAB) and socioeconomic indicators (IBGE). Results: In 2023, the Southeast region recorded 63,629 PrEP users, of whom 32,499 were new users initiated during that year. In the same period, the Northeast region accounted for 14,036 PrEP users, including 7,940 new users. This represents an absolute difference of approximately 49,593 users between the two regions, with the Southeast concentrating about 82% more users than the Northeast. In 2024, substantial growth was observed in both regions. The Southeast reached 99,333 PrEP users, with 48,615 new initiations, while the Northeast reached 22,323 users, including 11,464 new users. Despite the absolute increase in both regions, the relative proportion remained unfavorable to the Northeast, with the Southeast presenting approximately 77% more users in absolute numbers and a higher proportional growth rate. Analysis of Family Health Strategy coverage showed that the Northeast maintained population coverage above 80%, whereas the Southeast presented coverage of approximately 52%. These findings indicate that despite the greater presence of PHC teams in the Northeast, the recruitment and retention of PrEP users remain significantly lower. This suggests that technological infrastructure, the availability of specialized services, professional training, and sociocultural determinants exert a greater influence on PrEP access than the formal coverage of PHC teams alone. Additional factors that may explain this disparity include the concentration of HIV testing and counseling centers (CTCs) and specialized HIV/AIDS care services in the Southeast region, greater access to information and communication technologies for PrEP dissemination, stronger engagement of civil society organizations in mobilizing key populations, and lower levels of stigma associated with seeking HIV prevention services in more developed urban settings. These findings are consistent with the international literature, which highlights the existence of multilevel barriers to PrEP access, including individual factors (risk perception, stigma, and knowledge about PrEP), community factors (social norms and discrimination), and structural factors (service availability, professional training, and infrastructure). It is important to emphasize that the growth observed in both regions between 2023 and 2024 indicates that PrEP expansion strategies are producing positive results. However, the proportionally greater growth in the Southeast suggests that inequalities may be widening, reinforcing the need for targeted policies to accelerate implementation in the Northeast and other underserved regions. Continuous monitoring of these indicators is essential to assess the impact of interventions and guide adjustments in implementation strategies. Conclusion: Despite higher Family Health Strategy coverage, the Northeast region continues to exhibit significantly lower access to PrEP compared with the Southeast region. This finding demonstrates that the quantitative expansion of Primary Health Care alone is insufficient to ensure equitable access to HIV prevention technologies. Investments in professional training, infrastructure, intersectoral coordination, and the reduction of sociocultural barriers are necessary to promote more equitable access to PrEP throughout Brazil.
