Resumo
INTRODUÇÃO O câncer de colo de útero (CCU) constitui um importante problema de saúde pública. No Brasil, estimam-se 15,38 novos casos por 100.000 mulheres no triênio 2023–2025 (INCA, 2022). Embora diversos fatores estejam associados ao CCU, destaca-se o papel do papilomavírus humano (HPV) como principal agente carcinogênico. Os subtipos de alto risco 16 e 18 promovem a degradação dos supressores tumorais p53 e pRb, favorecendo a transformação maligna (Balasubramaniam et al., 2019). Entretanto, a infecção pelo HPV isoladamente não é suficiente para o desenvolvimento do câncer, sendo influenciada por cofatores, incluindo infecções do trato genital como a clamídia (Bray et al., 2024). A Chlamydia trachomatis (CT), bactéria intracelular obrigatória gram-negativa, está presente em cerca de 32% das mulheres com CCU (Pillai et al., 2022) e modula processos celulares relacionados à degradação proteica, apoptose e reparo do DNA (Yang et al., 2021). Dessa forma, busca-se compreender seu potencial papel na carcinogênese cervical. OBJETIVO Realizar uma análise comparativa do perfil de expressão gênica de mulheres com câncer cervical e infectadas por Chlamydia trachomatis, investigando mecanismos moleculares compartilhados. MÉTODO Perfis de expressão gênica foram obtidos de bancos de dados independentes da plataforma Gene Expression Omnibus (GEO/NCBI). Foram selecionados dois conjuntos de microarray: GSE9750, contendo 33 tumores primários, 9 linhagens celulares e 24 amostras de epitélio cervical normal; e GSE231775, composto por células endo e ectocervicais cultivadas como organoides 3D infectadas por CT (8 amostras). A análise de expressão diferencial foi realizada no software R utilizando o pacote limma. Após normalização dos dados e definição do desenho experimental, ajustou-se um modelo linear para comparação entre grupos. Os genes diferencialmente expressos foram definidos por log2 fold change = 1 e p ajustado < 0,05, com correção de Benjamini–Hochberg. Posteriormente, foi realizada análise de enriquecimento funcional utilizando listas ranqueadas derivadas das estatísticas do modelo linear. Foram empregados Gene Ontology (GO), KEGG e a coleção Hallmark da Molecular Signatures Database (MSigDB) para identificar vias biológicas e conjuntos gênicos enriquecidos, considerando normalized enrichment score (NES), false discovery rate (FDR) e significância estatística ajustada. RESULTADOS E DISCUSSÃO No banco de dados de câncer cervical, a comparação entre amostras tumorais e epitélio cervical normal identificou 13.516 genes, dos quais 389 estavam superexpressos e 343 subexpressos. Observou-se enriquecimento de processos relacionados à desregulação do ciclo celular, incluindo segregação cromossômica, replicação de DNA e sinalização de p53, enquanto vias associadas à queratinização e ao desenvolvimento da epiderme encontraram-se reduzidas, corroborando achados previamente descritos para o perfil transcriptômico do CCU (Elias, Das e Hamid, 2023). No conjunto de dados da infecção por CT, não foram identificados genes diferencialmente expressos estatisticamente significativos, possivelmente devido ao reduzido número de amostras e à elevada variabilidade biológica. Ainda assim, observou-se uma assinatura molecular compatível com intensa ativação imune e inflamatória. A análise Hallmark evidenciou ativação de programas relacionados à resposta mediada por interferons alfa e gama, à sinalização inflamatória dependente de NF-?B e à via IL6-JAK-STAT3. De forma consistente, análises em KEGG e Reactome confirmaram a ativação de processos envolvidos na comunicação entre células imunes, recrutamento leucocitário e mecanismos efetores da imunidade inata, sugerindo um microambiente inflamatório persistente. Adicionalmente, termos relacionados a cílios, axonema e microtúbulos encontraram-se enriquecidos, indicando remodelamento estrutural do epitélio infectado (Caven & Carabeo, 2023). A comparação entre os estados neoplásico e infeccioso demonstrou perfis biológicos distintos: o CCU apresentou predomínio de vias associadas à proliferação celular, enquanto a clamídia promoveu ativação imunológica e reorganização estrutural. Por outro lado, observou-se enriquecimento da via de transição epitélio-mesenquimal (EMT) na infecção por clamídia, sugerindo mecanismos de remodelamento epitelial. Esse achado é particularmente relevante devido ao papel da EMT na progressão tumoral da carcinogênese cervical, indicando possíveis mecanismos moleculares compartilhados entre a infecção por CT e o CCU (Igietseme et al., 2015; Lee e Shen, 2012). CONCLUSÃO Os resultados sugerem que a infecção por Chlamydia trachomatis promove uma resposta inflamatória robusta, caracterizada pela ativação de vias imunes e remodelamento epitelial. Esses achados reforçam a hipótese de mecanismos moleculares compartilhados entre a infecção persistente e processos envolvidos na carcinogênese cervical. IN SILICO ANALYSIS OF DIFFERENTIALLY EXPRESSED GENES AND MOLECULAR PATHWAYS IN CERVICAL CANCER AND GENITAL INFECTION BY Chlamydia trachomatis Keywords: Cervical cancer; Chlamydia trachomatis; Gene expression INTRODUCTION Cervical cancer (CC) remains a major public health concern. In Brazil, 15.38 new cases per 100,000 women are estimated for the 2023–2025 triennium (INCA, 2022). Although several factors are associated with CC, human papillomavirus (HPV) is recognized as the main carcinogenic agent. High-risk subtypes 16 and 18 promote the degradation of the tumor suppressors p53 and pRb, favoring malignant transformation (Balasubramaniam et al., 2019). However, HPV infection alone is not sufficient for cancer development and is influenced by cofactors, including genital tract infections such as chlamydia (Bray et al., 2024). Chlamydia trachomatis (CT), a gram-negative obligate intracellular bacterium, is present in approximately 32% of women with CC (Pillai et al., 2022) and modulates cellular processes related to protein degradation, apoptosis, and DNA repair (Yang et al., 2021). Therefore, understanding its potential role in cervical carcinogenesis is of great interest. OBJECTIVE To perform a comparative analysis of the gene expression profile of women with cervical cancer and women infected with Chlamydia trachomatis, investigating shared molecular mechanisms. METHODS Gene expression profiles were obtained from independent datasets available in the Gene Expression Omnibus (GEO/NCBI) platform. Two microarray datasets were selected: GSE9750, comprising 33 primary tumors, 9 cell lines, and 24 normal cervical epithelium samples; and GSE231775, consisting of endocervical and ectocervical cells cultured as 3D organoids infected with CT (8 samples). Differential expression analysis was performed in R using the limma package. After data normalization and experimental design definition, a linear model was fitted to compare the groups. Differentially expressed genes were defined by a log2 fold change = 1 and adjusted p-value < 0.05, using the Benjamini–Hochberg correction. Functional enrichment analysis was subsequently performed using ranked gene lists derived from the linear model statistics. Gene Ontology (GO), KEGG, and the Hallmark collection from the Molecular Signatures Database (MSigDB) were used to identify enriched biological pathways and gene sets, considering normalized enrichment score (NES), false discovery rate (FDR), and adjusted statistical significance. RESULTS AND DISCUSSION In the cervical cancer dataset, comparison between tumor samples and normal cervical epithelium identified 13,516 genes, of which 389 were upregulated and 343 downregulated. Enrichment was observed for processes related to cell cycle dysregulation, including chromosomal segregation, DNA replication, and p53 signaling, whereas pathways associated with keratinization and epidermal development were reduced, corroborating previous findings regarding the transcriptomic profile of CC (Elias, Das and Hamid, 2023). In the CT infection dataset, no statistically significant differentially expressed genes were identified, possibly due to the limited sample size and high biological variability. Nevertheless, a molecular signature consistent with intense immune and inflammatory activation was observed. Hallmark analysis revealed activation of programs related to interferon alpha and gamma responses, NF-?B-dependent inflammatory signaling, and the IL6-JAK-STAT3 pathway. Consistently, KEGG and Reactome analyses confirmed the activation of processes involved in immune cell communication, leukocyte recruitment, and innate immune effector mechanisms, suggesting the establishment of a persistent inflammatory microenvironment. Additionally, terms related to cilia, axoneme, and microtubules were enriched, indicating structural remodeling of the infected epithelium (Caven & Carabeo, 2023). Comparison between the neoplastic and infectious states demonstrated distinct biological profiles: CC showed a predominance of pathways associated with cellular proliferation, whereas chlamydial infection promoted immune activation and structural reorganization. On the other hand, enrichment of the epithelial–mesenchymal transition (EMT) pathway was observed in CT infection, suggesting epithelial remodeling mechanisms. This finding is particularly relevant given the role of EMT in cervical tumor progression, indicating possible shared molecular mechanisms between CT infection and CC (Igietseme et al., 2015; Lee and Shen, 2012). CONCLUSION The results suggest that Chlamydia trachomatis infection promotes a robust inflammatory response characterized by the activation of immune pathways and epithelial remodeling. These findings support the hypothesis of shared molecular mechanisms between persistent infection and processes involved in cervical carcinogenesis. REFERÊNCIAS BALASUBRAMANIAM, S.
