Resumo
ABORDAGEM FISIOTERAPÊUTICA EM CONDIÇÃO RARA E NEGLIGENCIADA: ESTUDO LONGITUDINAL DE PÚRPURA DE HENOCH-SCHÖNLEIN EM ADULTO Introdução A Púrpura de Henoch-Schönlein (PHS), ou Vasculite por IgA, é uma vasculite sistêmica predominantemente pediátrica. Na fase adulta, torna-se rara, mais severa e de diagnóstico desafiador, cursando com manifestações tegumentares, poliartralgia e acometimento gastrointestinal grave. No contexto da reabilitação, a PHS adulta é uma condição expressivamente negligenciada. A escassez de literatura focada em condutas fisioterapêuticas para esses pacientes dificulta o raciocínio clínico inicial, perpetuando o risco de agravos físicos e impactando diretamente na funcionalidade e vulnerabilidade social do indivíduo. Objetivo Descrever o raciocínio clínico e os efeitos de uma intervenção fisioterapêutica longitudinal (2022-2026) na recuperação motora, controle álgico e retorno ao esporte de alto rendimento de um adulto jovem diagnosticado com PHS. Método Estudo de caso retrospectivo e longitudinal. Paciente masculino, 25 anos, militar e atleta amador, apresentou quadro grave de PHS em 2022. O quadro clínico iniciou com petéquias disseminadas, evoluindo para lesões bolhosas e necrose inicial no dorso das mãos. Concomitantemente, manifestou queimação articular, rigidez em membros inferiores (MMII) e acometimento gastrointestinal severo (uso de sonda nasogástrica). Restrito ao leito por incapacidade de sedestação devido à dor articular, desenvolveu acentuada hipotrofia de quadríceps e tríceps sural, além de restrição articular em tornozelos. O protocolo fisioterapêutico hospitalar englobou imagética motora (dada a intolerância ao movimento ativo), exercícios metabólicos, respiração diafragmática e mobilizações passivas. O acompanhamento pós-alta (três vezes semanais) iniciou com drenagem linfática manual para edema secundário aos corticosteroides, associada à laserterapia de baixa intensidade (fotobiomodulação) no dorso das mãos. Com a progressão, instituiu-se eletroterapia analgésica, cinesioterapia para reabilitação tegumentar e articular das mãos, ganho de mobilidade de tornozelos e fortalecimento muscular progressivo. Nas fases finais, o foco direcionou-se ao retorno esportivo, com exercícios de dupla tarefa, treinamento sensoriomotor e condicionamento aeróbico de baixo impacto (bicicleta e esteira). Resultados e Discussão A avaliação inicial evidenciou dor máxima na Escala Visual Analógica (EVA 10/10). Com a intervenção, o quadro álgico regrediu para intensidade média a curto prazo pós-alta (EVA 5/10), estabilizando-se como dor mínima a longo prazo (EVA 2/10). A conduta inovadora de utilizar a imagética motora provou-se essencial desde a fase hospitalar até a fase de retorno ao esporte, pois permitiu ao paciente associar cognitivamente como o movimento deveria ser e como seria executado para a obtenção de uma melhor performance, além de garantir a manutenção da ativação cortical durante o período de imobilidade aguda. Ademais, embora o paciente tenha apresentado um quadro inicial de cinesiofobia devido ao receio da exacerbação álgica articular, ele demonstrou-se altamente disciplinado e colaborativo ao longo de todo o processo, o que contribuiu diretamente para o sucesso e a progressão segura do tratamento. Os exercícios metabólicos e a drenagem linfática mitigaram as complicações circulatórias e o edema decorrente das medicações autoimunes. A aplicação da laserterapia de baixa intensidade demonstrou eficácia clínica imediata na modulação inflamatória, aceleração da cicatrização das lesões bolhosas e reversão do processo de necrose inicial no dorso das mãos, restabelecendo a integridade tecidual por meio do estímulo à microcirculação local (AVCI et al., 2013). A longo prazo, a reabilitação reverteu a hipotrofia muscular dos MMII e a restrição dos tornozelos, recuperando a biomecânica da marcha e permitindo a restauração funcional completa das mãos. Superando a escassez de protocolos literários para a doença, a adaptação de princípios da fisioterapia esportiva e reumatológica culminou em um desfecho de excelência: o paciente retornou integralmente às atividades militares do exército e, atualmente, é maratonista amador. Conclusão A intervenção fisioterapêutica longitudinal demonstrou eficácia incontestável na reversão de incapacidades severas causadas pela PHS no adulto. O estudo evidencia que a fisioterapia, desde a fase hospitalar até o retorno ao esporte, associando recursos tecnológicos como a fotobiomodulação à condutas cinesioterapêuticas estruturadas, é uma ferramenta indispensável e transformadora no manejo dessa condição crônica e negligenciada, promovendo a reinserção profissional plena e a retomada da qualidade de vida. E-mail: cristianedelimadavilafisio@gmail.com Introduction Henoch-Schönlein Purpura (HSP), or IgA Vasculitis, is a predominantly pediatric systemic vasculitis. In adulthood, it becomes rare, more severe, and diagnostically challenging, presenting with integumentary manifestations, polyarthralgia, and severe gastrointestinal involvement. In the context of rehabilitation, adult HSP is a significantly neglected condition. The scarcity of literature focusing on physiotherapeutic management for these patients hinders initial clinical reasoning, perpetuating the risk of physical worsening and directly impacting the individual's functionality and social vulnerability. Objective To describe the clinical reasoning and the effects of a longitudinal physiotherapeutic intervention (2022-2026) on the motor recovery, pain management, and return to high-performance sports of a young adult diagnosed with HSP. Method A retrospective and longitudinal case study. A 25-year-old male patient, military personnel, and amateur athlete presented with a severe case of HSP in 2022. The clinical picture began with disseminated petechiae, evolving into bullous lesions and initial necrosis on the dorsum of the hands. Concurrently, he manifested generalized articular burning pain, lower limb stiffness, and severe gastrointestinal involvement (requiring a nasogastric tube). Bedridden due to the inability to sit up caused by joint pain, he developed marked hypotrophy of the quadriceps and triceps surae, as well as joint restriction in the ankles. The in-hospital physiotherapeutic protocol encompassed motor imagery (given the intolerance to active movement), metabolic exercises, diaphragmatic breathing, and passive mobilizations. The post-discharge follow-up (three times a week) began with manual lymphatic drainage for edema secondary to corticosteroids, associated with low-level laser therapy (photobiomodulation) on the dorsum of the hands. With progression, analgesic electrotherapy, kinesiotherapy for integumentary and articular rehabilitation of the hands, ankle mobility gains, and progressive muscle strengthening were introduced. In the final stages, the focus shifted to the return to sports, utilizing dual-task exercises, sensorimotor training, and low-impact aerobic conditioning (stationary bicycle and treadmill). Results and Discussion The initial assessment showed maximum pain on the Visual Analog Scale (VAS 10/10). Following the intervention, the pain regressed to moderate intensity in the short-term post-discharge period (VAS 5/10), stabilizing as minimal pain in the long term (VAS 2/10). The innovative approach of using motor imagery proved essential from the hospital phase through the return-to-sport phase, as it allowed the patient to cognitively associate how the movement should be and how it would be executed to achieve better performance, in addition to ensuring the maintenance of cortical activation during the acute immobility period. Furthermore, although the patient initially presented with kinesiophobia due to the fear of joint pain exacerbation, he proved to be highly disciplined and collaborative throughout the entire process, which contributed directly to the success and safe progression of the treatment. Metabolic exercises and lymphatic drainage mitigated circulatory complications and edema resulting from autoimmune medications. The application of low-level laser therapy demonstrated immediate clinical efficacy in inflammatory modulation, accelerating the healing of bullous lesions, and reversing the initial necrosis process on the dorsum of the hands, restoring tissue integrity by stimulating local microcirculation (AVCI et al., 2013). In the long term, rehabilitation reversed the muscle hypotrophy in the lower limbs and ankle restrictions, recovering gait biomechanics and allowing the complete functional restoration of the hands. Overcoming the scarcity of literature protocols for the disease, the adaptation of principles from sports and rheumatological physiotherapy culminated in an excellent outcome: the patient fully returned to army military activities and is currently an amateur marathoner. Conclusion The longitudinal physiotherapeutic intervention demonstrated undeniable efficacy in reversing severe disabilities caused by HSP in an adult. The study highlights that physiotherapy, from the hospital phase to the return to sports, combining technological resources such as photobiomodulation with structured kinesiotherapeutic protocols, is an indispensable and transformative tool in managing this chronic and neglected condition, promoting full professional reintegration and the restoration of quality of life.
