Resumo
Introdução. As taupatias compreendem um conjunto de doenças neurodegenerativas associadas ao acúmulo patológico da proteína tau, entre as quais se destacam a doença de Alzheimer (DA) e formas de degeneração lobar frontotemporal do tipo tau (DLFT-tau). Embora apresentem padrões relativamente característicos de atrofia, há sobreposição clínica e radiológica importante entre DA e DLFT-tau, especialmente nas fases iniciais, o que limita a acurácia do diagnóstico baseado apenas em avaliação clínica e inspeção visual de neuroimagens. Nesse contexto, a ressonância magnética (RM) estrutural e as técnicas de aprendizado de máquina têm emergido como estratégias promissoras para extrair biomarcadores de imagem capazes de aprimorar o diagnóstico diferencial dessas condições. Objetivo. Desenvolver e avaliar uma ferramenta computacional baseada em deep learning aplicada a neuroimagens de ressonância magnética estrutural para a identificação diferencial entre indivíduos cognitivamente normais, pacientes com DA e pacientes com taupatias do espectro frontotemporal. Método. Trata-se de pesquisa original em andamento, de caráter quantitativo e observacional, baseada em dados secundários provenientes de coortes multicêntricas de neuroimagem: Alzheimer’s Disease Neuroimaging Initiative (ADNI), National Alzheimer’s Coordinating Center (NACC) e ARTFL LEFFTDS Longitudinal Frontotemporal Lobar Degeneration Research Study/4-Repeat Tauopathy Neuroimaging Initiative (ALLFTD/4RTNI-2). Foram incluídos indivíduos com diagnóstico clínico e/ou neuropatológico consolidado de DA, taupatias do espectro frontotemporal ou condição cognitivamente normal, com exames de RM estrutural ponderada em T1 e qualidade adequada para análise volumétrica. A curadoria totalizou 642 imagens de DA, 642 de taupatias e 806 de controles. O pré-processamento incluiu skull stripping com SynthStrip, recorte de fundo, normalização espacial de intensidade e padronização volumétrica com o framework MONAI. Em seguida, foi treinada uma rede neural convolucional tridimensional baseada na arquitetura DenseNet169, com volumes padronizados em 192×192×192 voxels. Para reduzir vazamento de dados, a divisão foi realizada em nível de paciente, destinando-se 70% da base ao treinamento, 15% à validação e 15% ao teste. Também foram aplicadas técnicas de data augmentation tridimensional, ponderação da função de perda e análise de interpretabilidade por GRAD-CAM. Resultados e Discussão. Nas etapas iniciais, sem aumento de dados, o modelo apresentou tendência ao sobreajuste e maior viés em favor das classes mais representadas. Após a introdução de rotações, zoom tridimensional e ruído gaussiano, observou-se maior estabilidade de treinamento e redução da dependência da rede em relação à memorização da distribuição original dos voxels. O melhor modelo alcançou acurácia global de 71,7% em conjunto de teste independente com 421 volumes cerebrais. Para a classe normal, foram obtidos precisão de 68,49%, recall de 92,02% e F1-score de 78,53%; para taupatias, precisão de 77,78%, recall de 37,98% e F1-score de 51,04%; e, para DA, precisão de 74,10%, recall de 79,84% e F1-score de 76,87%. A matriz de confusão evidenciou melhor desempenho para controles e DA, enquanto a identificação de taupatias permaneceu mais desafiadora, possivelmente em razão da heterogeneidade neuropatológica desse grupo e da sobreposição morfológica com a DA. As análises de GRAD-CAM mostraram, em configurações iniciais, atenção indevida a crânio, pescoço e fundo, caracterizando shortcut learning; esse achado fundamentou o refinamento do pré-processamento anatômico. Em comparação com estudos multiclasse baseados em RM estrutural, a acurácia observada situa-se em faixa compatível com a literatura, embora ainda abaixo de cenários binários mais homogêneos. Conclusão. Os resultados demonstram a viabilidade do uso de deep learning em RM estrutural para apoiar o diagnóstico diferencial entre normalidade, DA e taupatias, com desempenho promissor sobretudo para controles e DA. A principal limitação concentrou-se na baixa sensibilidade para taupatias, indicando a necessidade de ampliar estratégias de balanceamento, aperfeiçoar a curadoria das classes e incorporar validações adicionais. Ainda assim, a abordagem apresenta potencial translacional como ferramenta de apoio à triagem e à priorização de casos para avaliação especializada, sem substituir a integração entre clínica, testes neuropsicológicos e outros biomarcadores. Tauopathies comprise a group of neurodegenerative disorders associated with the pathological accumulation of tau protein, including Alzheimer’s disease (AD) and tau-type frontotemporal lobar degeneration (FTLD-tau). Although these conditions may present relatively characteristic atrophy patterns, there is substantial clinical and radiological overlap between AD and FTLD-tau, especially in early stages, which limits the accuracy of diagnosis based solely on clinical assessment and visual inspection of neuroimaging. In this context, structural magnetic resonance imaging (MRI) and machine learning techniques have emerged as promising strategies to extract imaging biomarkers capable of improving the differential diagnosis of these conditions. Objective. To develop and evaluate a computational tool based on deep learning applied to structural MR neuroimaging for the differential identification of cognitively normal individuals, patients with AD, and patients with frontotemporal spectrum tauopathies. Method. This is an ongoing original quantitative and observational study based on secondary data from multicenter neuroimaging cohorts: the Alzheimer’s Disease Neuroimaging Initiative (ADNI), the National Alzheimer’s Coordinating Center (NACC), and the ARTFL LEFFTDS Longitudinal Frontotemporal Lobar Degeneration Research Study/4-Repeat Tauopathy Neuroimaging Initiative (ALLFTD/4RTNI-2). Individuals with a consolidated clinical and/or neuropathological diagnosis of AD, frontotemporal spectrum tauopathies, or cognitively normal status, along with available T1-weighted structural MRI scans with adequate quality for volumetric analysis, were included. Data curation yielded 642 AD images, 642 tauopathy images, and 806 control images. Preprocessing included skull stripping with SynthStrip, foreground cropping, spatial intensity normalization, and volumetric standardization using the MONAI framework. A three-dimensional convolutional neural network based on the DenseNet169 architecture was then trained with standardized 192×192×192 voxel volumes. To reduce data leakage, the dataset was split at the patient level into 70% for training, 15% for validation, and 15% for testing. Three-dimensional data augmentation, loss weighting, and GRAD-CAM interpretability analysis were also applied. Results and Discussion. In the initial stages, without data augmentation, the model showed a tendency toward overfitting and greater bias toward the most represented classes. After the introduction of rotations, three-dimensional zoom, and Gaussian noise, improved training stability and reduced dependence on memorizing the original voxel distribution were observed. The best model achieved an overall accuracy of 71.7% on an independent test set comprising 421 brain volumes. For the normal class, precision was 68.49%, recall was 92.02%, and the F1-score was 78.53%; for tauopathies, precision was 77.78%, recall was 37.98%, and the F1-score was 51.04%; and for AD, precision was 74.10%, recall was 79.84%, and the F1-score was 76.87%. The confusion matrix showed better performance for controls and AD, whereas the identification of tauopathies remained more challenging, possibly due to the neuropathological heterogeneity of this group and its morphological overlap with AD. GRAD-CAM analyses initially revealed undue attention to skull, neck, and background regions, characterizing shortcut learning; this finding supported refinement of the anatomical preprocessing pipeline. Compared with other multiclass studies based on structural MRI, the observed accuracy falls within a range compatible with the literature, although still below results reported for more homogeneous binary scenarios. Conclusion. Results demonstrate the feasibility of using deep learning on structural MR neuroimaging to support the differential diagnosis among normal aging, AD, and tauopathies, with particularly promising performance for controls and AD. The main limitation was the low sensitivity for tauopathies, indicating the need to expand balancing strategies, improve class curation, and incorporate additional validation procedures. Even so, the approach shows translational potential as a support tool for triage and case prioritization for specialist evaluation, without replacing the integration of clinical assessment, neuropsychological testing, and other biomarkers.
